Understanding IGF-1 DES: A Truncated Growth Factor With Reduced Binding-Protein Affinity
The defining characteristic of IGF-1 DES is its removal of three N-terminal amino acids. This modification changes interactions with circulating and tissue-associated IGF-binding proteins while preserving important receptor-related activity.
1. Native IGF-1
Insulin-like growth factor 1 is produced in the liver and many other tissues. It participates in development, protein metabolism, tissue maintenance, and growth-related signaling.
2. Removal of Gly-Pro-Glu
IGF-1 DES lacks the first three amino acids of mature IGF-1: glycine, proline, and glutamic acid.
The resulting 67-residue protein corresponds to native IGF-1 residues 4-70.
3. Naturally Occurring Processing
DES(1-3)IGF-I has been isolated from mammalian tissues. It is proposed to arise through proteolytic processing of native IGF-1.
4. Three Disulfide Bonds
IGF-1 DES retains the disulfide-linked protein structure associated with native IGF-1.
Correct folding is necessary for meaningful receptor activity.
5. IGF-Binding Proteins
IGFBPs regulate the availability, distribution, clearance, and actions of IGF-related growth factors.
They can both limit receptor access and prolong circulation through protein-complex formation.
6. Reduced IGFBP Binding
Removal of the N-terminal glutamate at native IGF-1 position 3 significantly reduces affinity for several IGFBPs.
The magnitude varies by binding protein and experimental assay.
7. IGFBP-3 Affinity
A biosensor study found approximately 25-fold weaker binding of des(1-3)IGF-I to recombinant IGFBP-3 compared with native IGF-1.
This does not imply an identical reduction in affinity for every IGFBP preparation.
8. Preservation of IGF1R Activity
The peptide retains important activity at the type 1 insulin-like growth factor receptor, a receptor tyrosine kinase.
9. Increased Apparent Potency
In laboratory conditions where IGFBPs limit native IGF-1 availability, DES may produce stronger biological responses at comparable concentrations.
Its increased potency does not necessarily reflect stronger intrinsic receptor binding.
10. Rat Granulosa Cells
A controlled cellular study found approximately tenfold greater activity for DES in stimulating FSH-associated progesterone accumulation when binding proteins were present.
When binding proteins were absent, DES and native IGF-1 had approximately similar potency.
11. IGF1R Autophosphorylation
Ligand binding can activate receptor tyrosine kinase signaling and intracellular phosphorylation events.
12. IRS and PI3K
Insulin receptor substrate proteins and phosphoinositide 3-kinase transmit signals involved in metabolism, protein turnover, and cell survival.
13. AKT
AKT signaling participates in regulation of cellular survival, glucose handling, and protein metabolism.
14. mTOR
The AKT-mTOR pathway is involved in protein synthesis and cellular growth.
Increased signaling in cultured cells is not proof of increased adult human muscle mass.
15. MAPK and ERK
IGF1R activation may also stimulate MAPK pathways involved in cell proliferation and differentiation.
16. Muscle Cell Biology
IGF-related signaling contributes to the normal biology of myoblasts, muscle adaptation, and tissue repair.
Direct human benefits from injected DES have not been established.
17. Satellite Cells
Satellite cells are muscle-associated stem cells that contribute to repair and adaptation.
Although IGF-related pathways influence myogenic processes, no controlled human study establishes satellite-cell benefits from DES administration.
18. Intestinal Tissue Growth
Animal studies have reported particularly noticeable anabolic activity in gastrointestinal tissues.
These results should not be interpreted as evidence of selective skeletal muscle growth.
19. Insulin-Like Glucose Effects
IGF-related signaling can influence glucose uptake and glucose production.
DES may produce clinically important glucose-lowering effects if sufficient systemic exposure occurs.
20. Animal Hypoglycemia Findings
Experiments in pigs and marmosets demonstrated potent, sustained hypoglycemic activity from low-IGFBP-affinity IGF variants, including DES.
21. Faster Clearance Despite Stronger Action
Reduced binding to circulating IGFBPs can cause faster clearance from plasma.
In animal studies, the glucose-lowering effect of low-binding variants was nevertheless more sustained than that of native IGF-1.
22. Local Versus Systemic Exposure
Injecting a growth factor near one tissue does not establish confinement to that tissue.
Claims of purely localized muscle growth require direct pharmacokinetic and clinical evidence.
23. DES Versus LR3
DES contains 67 amino acids, while LR3 contains 83 residues, including a 13-residue extension and an arginine substitution.
Both exhibit reduced IGFBP interactions, but they are structurally distinct and cannot be assumed to share pharmacokinetics or dosing.
24. DES Versus Mecasermin
Mecasermin is recombinant native human IGF-1 with 70 amino acids.
Its approved clinical effects and safety instructions cannot be directly transferred to DES.

Research takeaway: IGF-1 DES changes growth-factor availability by weakening IGFBP interactions. Its increased experimental activity is real, but it also has meaningful hypoglycemia findings in animals and no established human therapeutic safety margin.
