Orforglipron has demonstrated substantial clinical efficacy across multiple Phase 3 trials involving adults with obesity, overweight, and type 2 diabetes. Its results are strongest for body-weight reduction, HbA1c improvement, and selected cardiometabolic measurements.

ATTAIN-1 — Obesity Without Diabetes: The Phase 3 ATTAIN-1 trial enrolled 3,127 participants with obesity or overweight and at least one weight-related condition, excluding type 2 diabetes. After 72 weeks, mean weight reduction reached approximately 12.4% in the highest investigational dose group under the efficacy estimand, compared with approximately 0.9% for placebo.
Understanding Different Weight-Loss Numbers: The FDA prescribing label presents a treatment-regimen estimand that includes outcomes after treatment discontinuation and other intercurrent events. Under this analysis, the highest exposure group lost approximately 11.1% versus 2.1% with placebo. Both figures are valid within their respective statistical definitions and should not be combined as though they measure the same outcome.
ATTAIN-1 Categorical Weight Loss: In the highest exposure group of the FDA treatment-regimen analysis, approximately 71.5% achieved at least 5% weight loss, 54.5% achieved at least 10%, and 35.9% achieved at least 15%.
ATTAIN-2 — Obesity With Type 2 Diabetes: Participants with obesity or overweight and type 2 diabetes experienced significant reductions in body weight, along with improvements in HbA1c. The FDA treatment-regimen analysis showed approximately 9.6% weight loss at the highest exposure level versus 2.5% with placebo.
Blood Glucose and HbA1c: In ATTAIN-2, mean HbA1c fell approximately 1.7 percentage points in the highest exposure group under the FDA treatment-regimen analysis, compared with approximately 0.4 percentage points in the placebo group.
ACHIEVE-1: The Phase 3 trial investigated daily oral orforglipron for glycemic control in adults with type 2 diabetes. Investigational doses of 3, 12, and 36 mg produced clinically meaningful improvements in blood glucose and body weight.
ACHIEVE-3 — Direct Comparison With Oral Semaglutide: This 52-week trial randomized 1,698 adults with type 2 diabetes inadequately controlled with metformin. Mean HbA1c fell approximately 1.71 percentage points with orforglipron 12 mg and 1.91 points with orforglipron 36 mg, compared with 1.23 points with oral semaglutide 7 mg and 1.47 points with oral semaglutide 14 mg, using the treatment-regimen estimand.
ACHIEVE-3 Weight Comparison: Under the efficacy estimand, orforglipron 36 mg produced approximately 9.2% weight reduction versus 5.3% with oral semaglutide 14 mg. These findings establish superiority in the specific trial comparison, not superiority over all oral or injectable semaglutide formulations.
Weight Maintenance After Switching: The ATTAIN-MAINTAIN study examined adults who had previously lost substantial weight using injectable tirzepatide or semaglutide. Those assigned to orforglipron generally maintained more of their previously achieved weight reduction over 52 weeks than participants assigned to placebo.
Blood Pressure: ATTAIN-1 demonstrated reductions in systolic blood pressure. In the highest exposure group, the FDA analysis reported a mean decrease of approximately 6.4 mmHg versus 1.4 mmHg with placebo.
Triglycerides and Cholesterol: Clinical trials demonstrated favorable changes in triglycerides and selected cholesterol measures. These effects may reflect a combination of weight loss and other metabolic changes.
Body Composition: Orforglipron causes greater absolute fat-mass loss than lean-mass loss. However, loss of lean tissue remains a consideration, particularly during substantial weight reduction.
Cardiovascular Outcomes: Improvements in risk factors do not establish that orforglipron prevents heart attacks, strokes, or cardiovascular death. Such outcomes require adequately powered cardiovascular outcome research.
Research Perspective: Orforglipron has strong randomized clinical evidence for weight loss and improvements in selected metabolic outcomes. Its oral formulation offers an important practical difference from injectable therapies, but individual effectiveness, tolerability, and treatment suitability remain variable.
